At CIRCLE CARE Center, we continuously strive to give you outstanding medical care and quality service. New patients can speak with our Patient Advocate to discuss any insurance or billing-related issues. Our Patient Advocate can also help patients navigate through insurance co-pays, deductibles, co-insurance, and out-of-pocket expenses. All insurance-required co-pays must be paid at the time of your visit. With this cooperation, you will be able to receive all of the benefits offered to you, and we will be able to concentrate on caring for your medical needs.

CCC participates with Medicaid (Connecticut only), Medicare, AARP, ADAP (Connecticut only), AETNA, Anthem BC/BS, CIGNA, ConnectiCare, United Healthcare/Oxford, TriCare, MultiPlan, and several other commercial insurance policies.
CCC provides services to the uninsured. If you are uninsured, please let us know at the time you book your appointment so that you can speak directly with our Patient Advocate.

APPOINTMENT CANCELLATION POLICY
You must call 24 hours prior to your scheduled appointment time to avoid a mandatory $50 cancellation fee.
KNOWING YOUR INSURANCE Plan
It is important for you to understand your insurance plan, and it is your responsibility to know the limitations and benefits of your insurance coverage. It is extremely difficult for us to keep track of all the individual requirements of each insurance plan. Each one has different policies regarding how often services may be rendered and where those services may be performed. It is the patient's responsibility to know their policy. Within the same insurance company, the plans differ depending upon what type of contract you and/or your employer have negotiated. Eligibility for insurance payment is based upon the accuracy of the information you provide. Your current insurance and personal information are critical for billing. If you have changes in coverage or contact information, please be sure to inform us immediately.
