Patient Responsibilities

All patients of Circle Care Center are co-equal partners in their care. Patients have the responsibility to:
  • Follow Circle Care Center’s policies, procedures, and guidelines to help ensure a safe, affirming, and healthy environment for all. Not doing so may affect a patient’s ability to receive services at Circle Care Center, up to and including discharge from care.
  • Patients are required to keep track of your appointments and arrive on time.
  • Patients should actively participate in your care by:
    • Telling the provider what you need and about your concerns.
    • Working with the provider to decide on and plan treatment.
    • Asking questions when something is unclear.
    • Telling the provider if you are having trouble following the treatment plan.
    • Sharing accurate information with providers so they are best able to recommend treatment plans and other solutions.
  • Provide complete and accurate information about your current health complaints, past illnesses and medical history, hospitalizations, medications, allergies, and other matters relating to your health and/or mental health.
  • Patients who lie to obtain prescription medications put their care and the clinic at risk.
  • It is illegal for patients to sell or distribute medications prescribed to them.
  • Ensure that Circle Care Center has up-to-date contact information, including address, phone number, email, and emergency contact.
  • Patients are required to meet their financial responsibilities for the services received, which includes providing accurate health insurance information, providing proof of income, applying for government-sponsored benefits, and/or applying for patient assistance or payment plans. Circle Care Center charges a $25 fee for late cancellation (within 24 hour period) or no-shows.
  • Pay any copayments, coinsurance, deductibles, and other cost-sharing required by the patient’s insurers and any fees for non-covered services.
  • Patients authorize Circle Care Center to bill their insurer directly and assign to Circle Care Center all reimbursements from, and any legal or administrative claim against, the insurer.
In addition, all patients are required to abide by a code of conduct designed to ensure the safety of Care Circle Center’s clinical and office environment for additional patients and for our employees. All patients are required to:
  • Speak respectfully to Circle Care Center staff. Patients who raise their voice, curse at, or berate staff or other patients will be asked to leave, and may be discontinued as patients
  • Provide correct and updated insurance information. Patients are financially responsible for uncovered expenses.
  • Take medications as prescribed. Patients who misuse or sell medication may be discontinued as patients.
  • Avoid bringing weapons, alcohol, or other drugs (except for prescription drugs the patient may have a question for the provider or pharmacist about) into the facilities.
  • Respect the privacy and dignity of other patients.
  • Report any safety concerns to the clinic staff promptly.
