People who identify as transgender or gender non-conforming (TGNC) often face widespread discrimination and misunderstanding from health care providers. As a consequence, many transgender individuals do not have safe access to primary and preventive healthcare.
CIRCLE CARE Center provides a trans-affirmative environment where patients can receive hormone therapy as well as engage in an ongoing relationship with a primary care provider and/or a mental health provider to address the full spectrum of health and wellness needs.
CIRCLE CARE Center also provides care coordination tailored for transgender patients who need additional support. Information, assistance and referrals regarding benefits and entitlements, including insurance, housing, and referrals to other supportive services are available through Triangle Community Center (www.ctpridecenter.org).
TRANSGENDER HEALTH SERVICES INCLUDE:
- Hormone Replacement Therapy
- Sexually Transmitted Infection (STI) Screenings & Treatment
- HIV/AIDS Testing & Treatment
- Harm Reduction Counseling
- Pre-Exposure Prophylaxis (PrEP for HIV prevention)
- Mental Health Services
